Overview of Required Information to Place USAccess Equipment and Service Request Orders

The MSO recently updated the USAccess Service Request Order and Service and Equipment Order Forms. The updated forms are posted to the Customer Forms page on FedIdCard.gov. Additionally, all orders should now be emailed to the MSO Inbox (GSAMSO@gsa.gov) for review and placement. 

The MSO provides a variety of equipment and services to its customer agencies through orders. The table below was developed to facilitate USAccess Equipment, Service Request, test cards and technical support service ordering. Order types were grouped by equipment and service requests and include the required forms to accompany each type of order.

 
 

* One USAccess SET Worksheet is required for each USAccess Router. A single USAccess Router can support up to two FCU. If more than two FCU are being ordered for a single agency location, more than one USAccess SET Worksheet is required. If FCU are being ordered for multiple agency locations, USAccess SET Worksheets are needed for each location with different IP information.

** Technical Support Services involve Change Impact Analysis (CIA) and Change Request (CR) prices quoted by the Vendor to be approved by the Customer Agency and MSO.
